Deine Aufgaben
- Manage a Panel von Familien als Primary Care Provider
- Verbessere Whole-Family Health durch Longitudinal Primary und Preventive Care, Care Management, Care Coordination
- Coordinate Care im Home Setting und via Virtual Care Modalities (Video, Phone, SMS)
- Practice mit Support von Robust Interdisciplinary Team: MDs, LCSWs, RNs, Psychiatrists, Dieticians, Medical Assistants
- Utilize Nest Health's Electronic Health Record und Care Management Tools
- Attend Daily Team Huddles und Weekly Case Conferences
- Propose Treatments für Chronic und Infectious Diseases
- Prescribe und Reconcile Appropriate Medications
- Order, Administer, Obtain und Record Diagnostic Tests
- Provide Information und Assistance through Advocacy, Navigation Support, Education
- Participate in Outreach Efforts zu Nest Families
Deine Voraussetzungen
- Completion von Physician Assistant Program
- Active, Unrestricted License to Practice in Louisiana
- At Least 2 Years Clinical Experience mit Pediatric und Adult Patients
- Experience mit Chronic Medical und Behavioral Health Conditions
- Interest in Serving Underserved, Vulnerable Populations
