We are always building a pipeline of great talent to support our participants and our current team members Role Scope: We are looking for a dynamic, service-oriented physical therapist who is passionate about supporting elderly persons in their journey. The physical therapist will work within a multidisciplinary care team led by a primary care provider to ensure whole-person, integrated care. Participant Care: - Contribute to a center experience that Participants want to spend time in, a team culture that cares and creates joy, and an environment where all participants and team members belong. - Exhibit and honor Habitat’s Values. - Provides physical therapy evaluation and treatment on an acute, restorative and maintenance level to program participants. - Participates in Interdisciplinary Team (IDT) by contributing insights from assessments, care plan recommendations, and care coordination. - Assesses condition, creates a treatment plan and discusses implementation of therapy with participants and caregivers. - Makes home visits as needed and documents activities including participant/caregiver response in participant’s chart. - Educates participants and family members in proper care and use of various supports and exercise programs in addition to the use of DME. - Documents therapy services provided and performs department record-keeping in keeping within documentation requirements. Administration and Program Development: - Continue to raise the bar. Constructively seek and share feedback, and help us implement changes in order to improve clinical outcomes and experience for participants. - Assists in the ordering, inventory, distribution, and maintenance of durable medical equipment (DME) for participants. - Serves as subject matter expert for the development of physical therapy program standardization, evaluation and team education. - Serve as subject matter expert to support quality programs and discipline-specific audits. - Maintains the quality assurance, safety, environmental and infection control programs. - Performs data collection, prepares reports and maintains any necessary participant statistics. - Attends all staff meetings and patient care conferences as scheduled. Required Qualifications: - Bachelor’s Degree in Physical Therapy - Completion of a physical therapy program accredited by the American Physical Therapy Association (APTA), the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Physical Therapy Association or other equivalent organization approved by CMS - Active Physical Therapist license issued by California - A minimum of one year experience working with the frail or elderly. - Proof of medical clearance for communicable diseases, including a TB test. - Proof of all immunizations up to date. Preferred Qualifications: - 2+ years as a practicing PT - Bilingual: Spanish/Mandarin/Cantonese preferred. - Master’s Degree (MSPT) or Doctorate of Physical Therapy (DPT) - A state issued driver’s license, personal transportation, and auto insurance as required by law Location: Sacramento, CA (onsite) Compensation: We take into account an individual’s qualifications, skillset, and experience in determining final salary. Full time role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is $47.00 - $71.00 hourly. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
